Lessons from the NASA tragedy One of the ethical lapses by NASA is the ignorance of the NASA officials. From the case study it is realized that various parties had raised an alarm concerning the launch of the Colombia space shuttle. The space shuttle that was destroyed on its comeback from the space in January 16th 2003, despite the space agencies being aware of the complication that the shuttle had. Before the tragedy Gregory N.katnik noticed that the space shuttle had some damage on its ceramic tiles that resulted from a recent exploration. The engineer pointed out that the shuttle would not be protected from the earth intense heat because the foam debris was damaged. However, the space agency ignored the harm that could be caused by the form debris. Another act that caused the lapse of ethics by the NASA team is from the point that a mechanical design expert (Boisjoly) had warned that the shuttle was not ready for launch, only to be dismissed by his bosses. Poor communication mechanism between the organisations personnel s also contributed to the lapse of the organization ethics. The message that was communicated to the seniors by the juniors were dismissed and not put into consideration. The organization custom did not encourage bad news thus some risks were never communicated to the public or the officials. Ultimate cause of the lapse of the organization ethics is reliance on the past success (Jones, Walt, Tancredi & Broadribb, 2005). The NASA incidents shows that those who are at the higher ranks should be able to consider the views of their subordinates, it is noted that the technical experts had initially noticed the problem with the space shuttles but their concerns were dismissed by their seniors. Another factor for the tragedy is the poor communication between the seniors and the subordinates for instance the NASAs co-operate act of denial where serious problem are covered up and do not reach the seniors. The NASA incident suggests communication between the personnel is very vital. It as well indicates that the subordinates views should be taken into consideration by the managers as they are the ones who have the technical know how of what is happening (Jones, Walt, Tancredi & Broadribb, 2005). From the case study, it is realized that NASA can do the following to ensure that another incident similar to the ones mentioned in the article do not happen again. The first aspect is setting up a good relationship culture between the subordinates and the managers. Good relationship between the two parties will make it easy for vital information to be communicated to the seniors. The seniors to need to be taught on the importance of respecting the opinions of the subordinates who have the technical know how of the operations within the organization. The organization should as well set up ways of assessing and solving problems that are likely to occur when they launch a space shuttle. For instance the article shows that assessment of the challenger was not conducted very well, and if it was the findings were never put into consideration by the organization s management.
